Services

At Home is Best we approach all patients as individuals, carefully assessing the client’s physical, financial, social and psychological needs. We will work closely with caregivers and physicians to develop a personalized plan of care and assign a professional that best matches the needs of the client. The plan of care will outline the services, frequency and duration of visits, and will coordinate with your health care providers as necessary. After a caregiver is assigned, our agency performs routine follow-up visits at your home to make sure all expectations are being met.

Our home health services include:

Personal Home Care

Our compassionate, trustworthy and certified Home Health Aids and Personal Care Attendants are available to assist with dressing, eating, grooming, medication reminders and light housekeeping. They are trained in all areas of personal care needs, are CPR certified and have met all requirements for employment, including background checks. They work hand in hand with the client’s nurses, physician, and therapists to ensure that each patient’s care plan is carried out according to the wishes of the patient, their family and the patient’s physician. Our personal home care team is here to give you peace of mind, knowing that your loved one is in good hands.

Private Duty Nursing

Our caring, professional staff offers you or your loved one customized assistance and hands-on care, twenty-four hours a day. We provide a full spectrum of private care in the client’s private home, assisted or long term care facilities, rehab centers and hospitals.

Our staff will also assist with the personal care services needed to maintain independence in the home, including meal preparation, personal grooming, laundry, light housekeeping, medication reminders, companionship and caring for bed-bound clients. When one-on-one care is needed, we will be there for you, whether it’s two hour a day or for overnight stays or 24 hours a day.

Companion Care

Companion Care provides emotional support and companionship for seniors who are generally healthy and want to remain independent in their home. It can also include a range of non-medical services, including basic cooking, exercises, memory games, shopping, a ride to recreation centers, to a hair salon or to visit a friend or a loved one. It also includes cleaning and support that help make a senior’s life more manageable.

Respite Care

Caring for an elderly, chronically ill, or disabled family member may simply be a part of daily life, but it is a demanding role and no one is equipped to do it alone. It’s normal to need a break, that can benefit both the patient as well as the caregiver.

Our Respite Care Service provides short-term breaks that can relieve stress, restore energy, and promote balance in your life. Respite care can be for an ill or special needs adult or child, such as a child with autism or an adult with Alzheimer’s. Our services may be arranged for a weekend, a day, or even for an hour. Call us today and we will schedule respite care for you on a regular basis or just one time to give you the break you need.

Alzheimer's/Dementia Care

Our caregivers have the experience and training needed to provide appropriate, safe and individualized care in keeping with the distinct challenges often presented by patients living with memory impairments and associated behavioral issues. Our Alzheimer and Dementia home care program has been designed to maintain overall independence and quality of life in a loving and familiar environment.

Palliative Care

Caring for an terminally ill family member is a demanding role and no one is equipped to do it alone. Our staff is here to help provide a comfortable end-of-life experience, and an easier transition for your loved one and your family. At this stage in life, we care for the client, not the disease.

Our team will support the client’s physical, emotional and spiritual needs. We want our clients and their families to remain in the comfort of a familiar and peaceful setting, to approach the end of life with care and dignity.
